Provide the company’s name, registered office address, telephone number, email and VAT or official registration number. Provide every director’s full name, date and place of birth, address, and ID or passport copy. Provide all UBOs’ full names and, if possible, their dates and places of birth and addresses, together with their ID or passport copies. Belgian companies must provide a Belgian UBO register extract plus an additional supporting document; foreign companies must provide a document proving who their UBOs are. The letter exempts listed companies within the EEA, Australia, Hong Kong or the United States from completing its UBO section.

Declare whether any director or UBO is a PEP or related to one, and provide the requested title, position and source of financial means where applicable. If established in a high-risk country, as identified by FATF and the European Commission, provide the source of financial means and confirm that your credit institution applies due diligence standards no less strict than those required by Belgian anti-money laundering legislation.

Confirm that invoice payments will come from an account linked to the company and will not be made by a third party. Confirm the information and attached documents are truthful and correct to the best of your knowledge, then add the place, date and authorized signature.

Print on company letterhead. Answer whether you are a PEP and whether you or the entity are related to a PEP, using the definition provided in the form. If applicable, indicate the PEP profile and relationship. Confirm the information is complete and true to the best of your knowledge, then add your name, designation, date and signature.
